Birth Certificate (photocopy only)
Sf9 (Report Card)
Good Moral Character
Photo ID ( size 1x1 )

Go to the school website admission page or facebook page of our school then click the designated enrollment link or button for old, new or transferee.

Fill up the registration form (MLESF) then click submit.

Wait for the final posting of sections in our facebook page or message from your assigned class adviser.
On-site

Secure application form (MLESF) from the school c/o our Security Officer. General Clerk or grade level coordinator.
